Care Coordination

Across the region, care is coordinated for patients by a number of clinicians including:

  • general practitioners,
  • cancer support nurses,
  • acute based medical staff, and
  • allied health staff, to name a few.

There are significant opportunities to improve the coordination of care for patients and to improve integration of care between service providers.

Initiatives undertaken by LMICS include:

  • education, information and support to develop breast cancer support nurse roles into general cancer support roles, and the encouragement of a regional focus to these roles,
  • examination and improvement of service level opportunities, eg referral practices and documentation, and
  • investigation of the role of general practitioners and methods to support this role.
