Patient Management Frameworks

Patient Management Frameworks (PMFs) have been developed to provide a consistent statewide approach to care management in each tumour stream.

They are a clear description of each care pathway, identifying the critical points along that pathway and the optimal model of care required.

Fourteen PMFs have been developed. They are able to be downloaded from the Department of Human Services website as listed below:
